ANNUAL AFFIDAVITS:
The yearly pension affidavit must be notarized in order to continue receiving your pension benefit. Notarized affidavits can be scanned and e-mailed to: citypension@coj.net or mailed to the Pension Office.
ALL affidavits require notarization prior to delivery to an employee of the Pension Office and will require photo identification that reflects your current address.
